How to Obtain a Script for PT

1. Log on to your patient portal associated with your doctor’s office (i.e. UPMC MyChart)

2. Click “message provider” or “send a message”

3. Include a brief message stating your pain location, source and that you would like to start Physical Therapy

4. Ask for a prescription for “Evaluate and Treat” with Physical Therapist

5. Most doctors will send a PDF within 24-48 hours

6. Bring PDF copy to Summit Performance on your first visit.
